Parents and Guardians,
The end of the summer is getting closer, and the start of school is just around the corner. This also means that fall athletics and school activities are getting ready to resume. In order for your student to participate in athletics, there is some important information and dates that you should be aware of before practice begins.
Start of Practice:
The first day of practice for all fall sports is Monday, August 16th. Fall sports include: boys soccer, cheer/dance team, cross country, football, girls golf, and girls tennis. Specific start times and locations will be shared at a later time.
Mandatory Forms:
Students may not practice unless they have three pieces of paperwork on file in the OHS office. Please submit these forms by August 11th. These documents include:
Physical Exam: Students are required to have a physical on file each school year. The physical must be dated May 1st or after in the current year to be accepted. The form must be the specific physical form designated from KSHSAA. Most doctors have this form, but you can get additional forms from the OHS office or download it here: Physical Form
Concussion Form: Each student and parent must read and sign a concussion information sheet. Copies of this form can also be found at the OHS office or download it here: Concussion Form
Medical Consent Form: Students also need to submit a medical consent form that is signed by a parent in case of emergency. This form must be signed by a notary public. Our athletic administrative assistant is a notary public and can take care of this when you turn in the form. This form can also be obtained in the OHS office or download it here: Medical Consent Form
Please make sure you turn in these forms directly to the office. A student may not practice in any capacity until all paperwork has been completed and approved through the OHS office. These forms should not be submitted through a coach.
Impact Testing:
OHS conducts Impact Testing data collection at the beginning of the school year for all freshman athletes, junior athletes, and any other athlete that has had a previous concussion. This test is designed to generate baseline data that medical personnel can use to determine how quickly a student is recovering if they should happen to get a concussion. This test is for all athletes from all sports in all seasons that are freshmen, juniors, or student athletes that have had a previous concussion. This is not for fall sport athletes only. In order to complete the test, students will need to bring their school issued Chromebook because the test is completed online. Please make sure the Chromebook is charged and ready to test. The testing sessions are as follows:
Juniors: Tuesday, August 10th @ 3:45pm in the OHS Cyclone Room
Freshmen: Thursday, August 12th @ 9:00am in the OHS Science Exploratorium. Students may enter the front doors of the school to get to the Science Exploratorium.
Open Session: Thursday, August 12th @ 3:45pm in the OHS Science Exploratorium. Any freshman or junior athlete that cannot make it to their assigned session may attend the open session.
Any athlete that has had a previous concussion that is not a freshman or junior may attend any of the three sessions. If you have a freshman and junior student in your household, they both may attend one of the sessions together if that is the most convenient for your family. Similarly, we would like for Juniors and Freshmen to come to their specific sessions, but students may attend another session if that is the only time that will fit your schedule.
These tests typically take around 45 minutes to 1 hour to complete, This may vary and be slightly longer or shorter for some students.
